Employees must submit a Hybrid Work Agreement. By submitting this Hybrid Work Agreement, you acknowledge that you have read the Hybrid Work Policy 2-38 and agree to all items covered in the policy.
Additionally, employees may request to borrow office equipment such as desk chairs, monitors, keyboards and/or other ergo equipment through the Equipment Loan form.
